    Iniqüidades sociais na saúde e nutrição de crianças em países de renda baixa e média

    OBJECTIVE: To describe the effects of social inequities on the health and nutrition of children in low and middle income countries. METHODS: We reviewed existing data on socioeconomic disparities within-countries relative to the use of services, nutritional status, morbidity, and mortality. A conceptual framework including five major hierarchical categories affecting inequities was adopted: socioeconomic context and position, differential exposure, differential vulnerability, differential health outcomes, and differential consequences. The search of the PubMed database since 1990 identified 244 articles related to the theme. Results were also analyzed from almost 100 recent national surveys, including Demographic Health Surveys and the UNICEF Multiple Indicator Cluster Surveys. RESULTS: Children from poor families are more likely, relative to those from better-off families, to be exposed to pathogenic agents; once they are exposed, they are more likely to become ill because of their lower resistance and lower coverage with preventive interventions. Once they become ill, they are less likely to have access to health services and the quality of these services is likely to be lower, with less access to life-saving treatments. As a consequence, children from poor family have higher mortality rates and are more likely to be undernourished. CONCLUSIONS: Except for child obesity and inadequate breastfeeding practices, all the other adverse conditions analyzed were more prevalent in children from less well-off families. Careful documentation of the multiple levels of determination of socioeconomic inequities in child health is essential for understanding the nature of this problem and for establishing interventions that can reduce these differences.OBJETIVO: Describir el efecto de las iniquidades sociales sobre la nutrición y salud de niños de países de renta baja y media. En investigación realizada en la base PubMed, en el período de 1990-2007 fueron encontrados 244 artículos relacionados al tema. Fueron también analizados los resultados de cerca de 100 pesquisas de ámbito nacional recientes, incluyendo Pesquisas Nacionales de Demografía y Salud y Pesquisas por Conglomerados de Múltiples Indicadores, del Fondo de las Naciones Unidas para la Infancia. RESULTADOS: Niños de familias pobres, en comparación con aquellas de familias más ricas, son más susceptibles a la exposición a agentes patogénicos; una vez expuestas, tienen un riesgo aumentado de enfermar, debido a su menor resistencia y menor cobertura de medidas preventivas. Una vez que se toman enfermos, tienen menor acceso a servicios de salud, la calidad de los servicios que logran utilizar tiende a ser inferior, con menor acceso a tratamientos médicos que garanticen su sobrevivencia. Como consecuencia, niños de familias más pobres presentan mayores tasas de mortalidad y mayor riesgo de ser subnutridas. CONCLUSIONES: Excepto obesidad infantil y prácticas inadecuadas de amamantamiento, todas las otras condiciones adversas analizadas tuvieron mayor prevalencia entre los niños de familias menos favorecidas.     Focus and coverage of Bolsa Família Program in the Pelotas 2004 birth cohort

    OBJECTIVE To describe the focalization and coverage of Bolsa Família Program among the families of children who are part of the 2004 Pelotas birth cohort (2004 cohort). METHODS The data used derives from the integration of information from the 2004 cohort and the Cadastro Único para Programas Sociais do Governo Federal (CadÚnico – Register for Social Programs of the Federal Government), in the 2004-2010 period. We estimated the program coverage (percentage of eligible people who receive the benefit) and its focus (proportion of eligible people among the beneficiaries). We used two criteria to define eligibility: the per capita household income reported in the cohort follow-ups and belonging to the 20% poorest families according to the National Economic Indicator (IEN), an asset index. RESULTS Between 2004 and 2010, the proportion of families in the cohort that received the benefit increased from 11% to 34%. We observed an increase in all wealth quintiles. In 2010, by income and wealth quintiles (IEN), 62%-72% of the families were beneficiaries among the 20% poorest people, 2%-5% among the 20% richest people, and about 30% of families of the intermediate quintile. According to household income (minus the benefit) 29% of families were eligible in 2004 and 16% in 2010. By the same criteria, the coverage of the program increased from 43% in 2004 to 71% in 2010. In the same period, by the wealth criterion (IEN), coverage increased from 29% to 63%. The focalization of the program decreased from 78% in 2004 to 32% in 2010 according to income, and remained constant (37%) according to the IEN. CONCLUSIONS Among the families of the 2004 cohort, there was a significant increase in the program coverage, from its inception until 2010, when it was near 70%. The focus of the program was below 40% in 2010, indicating that more than half of the beneficiaries did not belong to the target population.OBJETIVO Descrever a focalização e a cobertura do Programa Bolsa Família nas famílias de crianças que fazem parte da coorte de nascimentos de Pelotas, 2004 (coorte de 2004). MÉTODOS Os dados utilizados derivam da integração de informações da coorte de 2004 e do Cadastro Único para Programas Sociais do Governo Federal, no período de 2004 a 2010. Estimamos a cobertura do programa (percentual de elegíveis que recebem bolsa) e seu foco (proporção de elegíveis entre os beneficiários). Utilizamos dois critérios para definir elegibilidade: a renda familiar per capita relatada nas avaliações da coorte e pertencer aos 20,0% mais pobres pela classificação do Indicador Econômico Nacional, um índice de bens. RESULTADOS Entre 2004 e 2010, a proporção de famílias beneficiárias da coorte passou de 11% para 34%. Houve aumento em todos os quintis de riqueza. Em 2010, por quintis de renda e Indicador Econômico Nacional, 62%-72% das famílias eram beneficiárias entre os 20% mais pobres, 2%-5% entre os 20% mais ricos, e cerca de 30% das famílias do quintil intermediário. Pelo critério de renda familiar, excluindo-se o valor do benefício do programa, 29% das famílias eram elegíveis em 2004 e 16% em 2010. Pelo mesmo critério, a cobertura do programa passou de 43% em 2004 para 71% em 2010. No mesmo período, pelo critério de riqueza (Indicador Econômico Nacional), a cobertura passou de 29% para 63%.     Nuclear Weapons in Regional Contexts: The Cases of Argentina and Brazil

    South America is a region which is free from nuclear weapons. However, this was not an inevitable development from the relationships among its countries. Indeed, regional rivalries between Brazil and Argentina, with military implications for both countries, lasted a long time. After WWII these countries took part in the race to obtain nuclear technologies and nuclear ambitions were part of the game. In the mid 1980s, the end of military dictatorships and the successful establishing of democratic institutions put an end to the race. Thus regional and national interests in addition to the establishment of democracies in Latin America have been responsible for the building of trust between the two countries. Meaningful international initiatives are once again needed in the framework of worldwide cooperation.     Gravidezes subseqüentes: quem as tem e quem as quer? Observações em um centro urbano da região Sul do Brasil

    Em um estudo de coorte de base populacional, foram estudados todos os 6.011 nascimentos hospitalares ocorridos na cidade de Pelotas, RS, em 1982. As parturientes foram entrevistadas logo após o parto, e 80% delas foram novamente contactadas em suas residências no início de 1986, em média 43 meses mais tarde. Dessas mulheres, 39% haviam engravidado novamente. Esta proporção variou inversamente em relação à idade materna, anos de escolaridade e renda familiar. Quanto à paridade, a proporção de gravidezes subsqüentes foi maior para primíparas e para multíparas. Mães cujos filhos nasceram através de cesareana também apresentaram menor fecundidade, mesmo após exclusão daquelas que, por ocasião da operação cesárea, sofreram ligadura de trompas. Análise através de regressão logística mostrou que esses fatores permaneceram significativamente associados à fecundidade mesmo após o ajuste estatístico para as demais variáveis. Das mães que engravidaram após 1982, 60% informaram que não a desejaram. A proporção das gravidezes indesejadas foi mais elevada em mulheres de maior paridade, sendo esta tendência mais marcada em mulheres de famílias de alta renda.Subsequent pregnancies in mothers of a birth cohort from Pelotas, Southern Brazil, were studied in relation to maternal and socio-economic factors. Within about 3 1/2 years of the cohort child's birth, 39% of mothers had experienced at least one further pregnancy. This proportion decreased with increasing maternal age, years of schooling and family income. A U-shaped trend was observed with respect to parity. Mothers who had delivered the cohort child by caesarean section were also less likely to have another pregnancy within that time. Logistic regression analysis showed that each of these factors remained significantly associated with further pregnancies after controlling for the remaining variables. Analysis of the first subsequent pregnancy showed that a high proportion of mothers had not wanted the pregnancy. Unwanted pregnancies were also significantly associated with older women, low educational status, higher parity and low family income

    A Transcriptomic Approach to Understanding the Combined Impacts of Supra-Optimal Temperatures and CO2 Revealed Different Responses in the Polyploid Coffea arabica and Its Diploid Progenitor C. canephora

    Understanding the effect of extreme temperatures and elevated air (CO2) is crucial for mitigating the impacts of the coffee industry. In this work, leaf transcriptomic changes were evaluated in the diploid C. canephora and its polyploid C. arabica, grown at 25 C and at two supra-optimal temperatures (37 C, 42 C), under ambient (aCO2) or elevated air CO2 (eCO2). Both species expressed fewer genes as temperature rose, although a high number of differentially expressed genes (DEGs) were observed, especially at 42 C. An enrichment analysis revealed that the two species reacted differently to the high temperatures but with an overall up-regulation of the photosynthetic machinery until 37 C. Although eCO2 helped to release stress, 42 C had a severe impact on both species. A total of 667 photosynthetic and biochemical related-DEGs were altered with high temperatures and eCO2, which may be used as key probe genes in future studies. This was mostly felt in C. arabica, where genes related to ribulose-bisphosphate carboxylase (RuBisCO) activity, chlorophyll a-b binding, and the reaction centres of photosystems I and II were down-regulated, especially under 42 C, regardless of CO2.     COVID-19 and social distancing among children and adolescents in Brazil

    OBJECTIVE To estimate the prevalence of SARS-CoV-2 antibodies and the adherence to measures of social distancing in children and adolescents studied in three national surveys conducted in Brazil between May–June 2020. METHODS Three national serological surveys were conducted in 133 sentinel cities located in all 27 Federative Units. Multistage probability sampling was used to select 250 individuals per city. The total sample size in age ranges 0–9 and 10–19 years old are of 4,263 and 8,024 individuals, respectively. Information on children or adolescents was gathered with a data collection app, and a rapid point-of-case test for SARS-CoV-2 was conducted on a finger prick blood sample. RESULTS The adjusted prevalence of antibodies was 2.9% (2.2–3.6) among children 0–9 years, 2.2% (1.8–2.6) among adolescents 10-19 years, and 3.0% (2.7–3.3) among adults 20+years. Prevalence of antibodies was higher among poor children and adolescents compared to those of rich families. Adherence to social distancing measures was seen in 72.4% (71.9–73.8) of families with children, 60.8% (59.6–61.9) for adolescents, and 57.4% (56.9–57.8) for adults. For not leaving the house except for essential matters the proportions were 81.7% (80.5–82.9), 70.6% (69.6–61.9), and 65.1% (64.7–65.5), respectively. Among children and adolescents, social distancing was strongly associated with socioeconomic status, being much higher in the better-off families. CONCLUSIONS The prevalence of antibodies against SARS-CoV-2 showed comparable levels among children, adolescents, and adults. Adherence to social distancing measures was more prevalent in children, followed by adolescents.     Survey and characterization of corbelled dome architecture in Northwestern Portugal

    The act of overlaying stones to build masonry structures is one of mankind’s most intuitive and resourceful achievements. Masonry buildings can be found all over the world, built with a wide diversity of materials and serving for all kind of purposes. In rural northwestern Portugal, in a granitic highland landscape of scarce resources, for centuries, local farmers used masonry in order to make the territory suitable for farming. In their effort to increase production and, by doing so, to improve their living conditions, a specific mountain agro-pastoral system was developed. Supported by communitarian labour, this system was based on terrace farming and temporary mountain plateau settlements, called brandas, composed of granite corbelled dome buildings. In spite of its perfect adaptation to the harsh local conditions, this heritage’s morphological and typological diversity and constructive inventiveness is endangered due to decades of rural exodus and, thus requiring urgent study and protection. The research project aims to study this heritage from morphological, typological and constructive points of view, with the objective of proposing guidelines towards its preservation. To achieve the established goals, a four stage based methodology was established. Literature review and fieldwork surveys were performed in the first stage, in order to identify possible case studies. On the second stage, an exhaustive geometrical survey and a case study analysis were undertaken and complemented with the local granite’s physical and mechanical characterization. The third stage aims to do an overall structural safety evaluation of the corbelled domes, based on numerical analysis. In the final stage, consolidation and preservation guidelines to safeguard this architectural heritage will be proposed.
